The reason people assume it's a render is simply because since it is posted on blendernation it is automatically assumed to be a demonstration of some super-awesome rendering technique (which it is: it's called photography). Going by the original forum post, I'd guess that the Suzanne model on top was lovingly crafted out of putty (so it's not actually edible). Even still, this is an awesome bit of work- I'm not sure myself if it would have been easier to do in blender or in the real world! Ok, so as far as I can tell, the cake is real.
